This started happening to me after I went and got the free Rayfield Caliburn from that tunnel where you fight Nash. The legendary tank top there was unleveled, and I didn't think anything of it until all of the NCPD Assault in Progress side gigs I've done stopped giving leveled loot.
I know that occasionally enemies will have un-leveled guns, but I've never encountered this before where the loot I'm getting is basically for a level 1 player when I'm at level 28.
Restarting the game does fix this issue, or at least it did on one occasion. I need to test it a second time to be sure.
