Probably no, although there will apparently be some minor new dialogue about Geralt's mark in the DLC with Ciri, Triss, and Yennefer (2-3 lines with each), most likely available during the Final Preparations quest (level ~28-30). The expansion quest line itself is designed to be playable at any time, but it is recommended for level 30 and above, that is why I guess it is ideally done in act 3 of the main quest after returning to Novigrad from the Bald Mountain.
I may (and would like to) be wrong about main characters not being present in the expansion pack, but integrating it into the open world design with the possibility of playing the expansion quests at any time makes adding them fairly difficult for the developers; see this post for an example why. It would be easier with an expansion that is either a separate smaller game (with something like "New Game - Blood and Wine" added to the main menu), or one that can only be played after the main quest is completed. For similar reasons, I do not think the expansion quests will have any significant consequences on the main game.

Interesting, i can imagine "Blood and Wine" take place after W3 main story now, we can import the ending's save to see different outcome in the expansion

My biggest hope is CDPR are doing this huge patch to add all the main characters back into the game post story hence the new dialogues.

So, apparently both expansions will be integrated with the vanilla game. While it is not certain this will be the case, but I think it looks likely now that the answer to the OP's question is "no" for both HoS and BaW, other than some extra dialogue or other minor additions.

Definitely a no. But I'd love to see what kind of romance content do the expansion have in store for Yen/Triss hmm.
I did suggest there could be a way to 'reconcile' with Yen and Triss if we choose to play the love triangle but it's already considered as a post-game content (complex issue but let's just say it won't happen, at least according to the description of the two upcoming expansion. Any fixes at this point will require a new expansion if CDPR takes into account of the 'immersion'). Plus, I highly doubt CDPR will listen to something insignificant anyway.
