Witcher 2 Chopping off Limbs and kill cut scenes

PROLOGUE AND CHAPTER 1 SPOILERS IN MY QUESTION

when i first played the witcher 2 i was able to chop off a soliders arm. this occurred in the prologue when geralt had to fight off la vallette soldiers to shoot the ballista at the la vallette tower entrance. i do not remember if i did anything special i just used the sword and i think the final blow before the soldier died his arm popped of and there was a little blood squirt and he fell to the ground and died. i have never once been able to do this again after playing the games 3 times in total. does anyone know what cuased this or how to do it again? also in flotsam i got attacked by a group of humans in the forest and i did about 5 awesome looking kill cutscene finisher moves. does anyone know what caused those to happen back to back like that? they usually happen to me on occasion, i am guessing aard is what causes those to happen. if not any suggestion on how to make them happen more often?

They happen when you hit someone stunned by Aard

Aard sign, preferably upgraded to the max.
Samum bomb and Zerrikanian Sun bomb.
Also Combat Acumen skill let you perform finisher on up to three opponents at once, when your adrenaline bar is full.

I have noticed this as well, and I think the La Valette soldiers are special to allow for this kind of dismemberment; probably in an attempt to appeal to gore-lovers early on in the game, so that they won't suddenly stop playing or something. I can't recall ever seeing dismemberment for any other type of enemy, further on in the game.

As people have pointed out already, you get this 'finisher move cutscene' if you attack a foe that is either stunned or knocked down (or if you use the Group Finisher adrenaline ability, at the end of the Swordsman tree). The Aard sign can cause both knockdown and stun, but also some non-sword weapons have especially high critical effect chance to cause these status effects.
