I have never liked the automatically failing on a one. The latest house rule we are using is:
Roll a 1 then roll again.
On a 1 to 2 you hesitate, add 0.
On a 3 or 4 you misstep, subtract 5.
5 to 6 you fail, no fumble.
7 is a misfire/fail and take 1 damage.
8 weapon jams/fail and take...
