How to start CAVE OF DREAMS side quest?

Where to find ghosts ? I found some in 1 cave, with level 30 ghosts? And want to find them again, any idea where? Skellige of course.

Which quests are missable? Still in Velen & Novigrad area and there's too much to do....some quests are grayed, some even failed :( So how's the right approach? MAIN, SIDE, WITCHER, TREASURE... i know treasure can wait. etc.....but always problems with main and side.

White Orchar Mislav quest When i go to commander Peter Saar Gwynleve, garrison commander in Nilfgaardian Camp....it doesn't offer me TOMIRA or MISLAV?
