I'm getting an automatic failure for El Capitan's "Just Another Story" missions. Specifically, when I cross into the intersection in the screen capture. It has happened a couple of times coming from Dog Town to a variety of drop-off locations on the Northside of Night City. The intersection in on "Broad" Street right after the bridge.
They also seem to occur on timed missions, neutralize missions, and good-condition missions.
