If you gotto the warehouse that has the nightclub in it from the quest investigating the death of Mayor Holt before doing the quest and enter the elevator the player is put on a endless loading and falliing through the map cycle.
If you go to the house where you meet judy for Pyramid Song again before you even reach the quest, the box with the diving suit is there which the player can loot and put on but cannot ever remove and fully locks them out of there equipment, i never played it long enough to see if it breaks the actual quest aswell

Subtitles - specifically Johnny's constantly being stuck on screen, i have a playthrough where no matter what i do wether it's hard reset xbox, shut game down, reload saves, turn off all subtitles everytime i load it up he says "ha, house is raking it in wonder what the boss says about it" and even if all subtitles are turned off that line constantly stays on my screen, very annoying
