Likely an issue with some equip tags in their editor. When you take your pants off in full nudity mode, you are fully naked in the menu, but you have boxers/panties on ingame.

And if you are in V's apartment and have pants equipped, they disappear when you look in the mirror. The pants are possessed I tell you.
