In order to have risk and stakes, you have to have friction. Friction provided by an opposition, something modern media is forgetting what with how Star Wars barely even has any threat or substance to their antagonists.
You got to feel the weight of the world, for it to have any meaning that you're standing out against it.
Assuming you as the player in an open RPG choose to stand out in it. I would love to revel in the dirt in some playthroughs making ends meet someway, start out trying to survive as a prostitute even, before finding your way to rise up in the world.
Games underestimate how interesting it can be to explore baser existences that aren't god-emperor savior of mankind, and a proper sandboxy RPG should be able to let you explore those elements. Let you go at your pace and eventually push past it to strive for heroism- Or villainy.
Yeah, I hear you. I think you'll get what you want (to a degree - I don't know about playable prostitution), as CDPR has already promised a "street level" story and not so much a big rise to power/fame like what you'd see in the Elder Scrolls or a Bioware RPG.
