For the advancement of learning quest problem

I completed this quest and send Keira to Kaer Morhen, but when i go to te Isle of Mists, the game is saying that, the quest has failed. Any idea?

Are you sure you didn‘t accidentally send her to Radovid? Did you check out the journal entry regarding this quest?

The only way to change things like that on PS4 is to reload an earlier save game - but I doubt you‘ll wanna do that, as it would be a huge set back.

Yes, im sure I send her to Kaer Morhen, she walked across the portal too.
