what is the cause of this texture weird thing and what can I do to remove it beside start over and hope it does not happen again ?

I had encounter this issue sometime with texture flying around after exit vehicle, by re-equip the top will remove the issue but then it comes back again, and this happen only in some save files (I had start over around 8 times w/o getting further than act 1)

is there a way to solve the issue other than not wearing the top ?
