Ugly graphics glitch - Low detail terrain popping?

In certain areas of the game, I get what looks, to me, to be extremely long range, low LOD versions of the terrain popping in for a single instant then disappearing. It is extremely ugly looking and ruins the visuals when it occurs. It only happens when moving the character or the camera, and while it can be made to happen repeatedly, the effect itself is extremely brief.

Many areas of the game are immune to it. I've never seen it happen indoors, reinforcing my belief that I'm seeing low detail versions of the terrain. In some locations it is very minor, but some, like one of the hills near Flotsam, it can be extreme.

I cannot take screenshots of it because the actual issue only persists for a frame or two. I cannot take videos of it because running Fraps in video capture mode actually stops it from happening.

Any help would be appreciated.

System:

Windows 7 64 bit
Quad Core AMD Phenom II at 3700 mhz
8 GB RAM
AMD Radeon HD 7850 1 GB
 
A few little extra bits of info I just thought of to add.

-I'm running the Steam version
-I'm running the latest version of the Catalyst drivers (19.1)
-On the suspicion that it may have been a driver issue related to my recent switch from an NVidia card, I just uninstalled my drivers, ran driver sweeper on everything video related, and then reinstalled them.
-No other game I have installed is having any graphic related issue.

I have tried everything. I have GTX 570, latest drivers, all updated. Fresh windows 7 x64, all updated...
