Broken Flowers glitch

Whenever I play the mission 'broken flowers' I get to the bit where you need to duel Rosa Var Attre, and then she disappears after the first round of fighting, i can't mediate, open the map or leave, and have no choice but to open up a previous save game, losing lots of time.
Is there a quick fix for this? Has anyone else experienced this or something similar? I'd be greatful for any help/advice anyone can give.

I have three glitches with this quest. I've done the quest twice and only one of them happened both times.

1. I can't be very specific about which dialogue options because I don't remember, but I think it was with Molly. There are two yellow options. I selected the first one and it was still there and yellow when it came back to the options list. I chose the second one, and when coming back to the list the *first* one went away. I then had to select the second one again to get it to go away and proceed to the next set of dialogue.

2. When Rosa wants a rematch, the fight starts while Geralt is not controllable and she hits me no matter what. This is the one that happened each time for me. Maybe this is intentional though.

3. In the very last scene of the quest, the NPC is duplicated in the background:

Starting around 10:10. Then at 10:49 you can see both options are yellow again.
