In Number 3 my Delamaine jr decided to spawn inside a locked garage in one of the Badlands missions and lost a few accessories punching through to get to me. For the rest of the play session it would turn up looking like this even if it respawned Next day it was back to pristine.

The high fashion lime green pants with orange legionnaires cap is a little unusual i'll give you but at level 36 i've crafted this set up to 1259 armour so I'll put up with the odd cry of "my eyes! my eyes!"
