DO NOT reload your previous save file once you opened treasure chest
Now I don't know whether this problem comes from the latest patch 1.03 on PS4, but here goes....
I can say this with absolute guarantee that there is a major glitch in Witcher 3 where if you reload your previous save file once you open a treasure chest, the same treasure chest will not give you what it intended you to have. It just gives you tresh items to add to your collection of riches.
What this means is, you CANNOT reload your previous file once you open a particular treasure chest because if you do, what I said above will happen.
If you doubt me, try testing it out yourself
