how do i fix this its running at medium and with nothing more i have no mods i don't know what is wrong could you help me please

Something on your end. I have a 3600 12gb 1080p with most setting on high, Dlss on balanced and rt off. run at usually around 60 fps at all times.

Make sure to run DLSS, quality or balanced should do.
Also you forgot one of the most important info, your screen resolution. 4K will even have a hard time with 3070Ti.
I had a "2070 super" in 2020/2021 and now have a "3080 Ti" and even with the 2070 I could get >40fps with raytracing + DLSS; on 2560x1440.
