How random is item placement?

How random is item placement?

If I were searching through an NPC's belongings and found something that seemed odd, like a particular book or a doll, is it just a random event or should I bear it in mind when I make later decisions? Or is stuff only important if Geralt comments on it?

Anyone from RED around? This could have an impact on a choice I make..

I think that even if you loot it, it won't matter later, just as long as it is still in the inventory

It's not about looting.. it's about a character having a particular item amongst their things and I don't know if it's just a random accident or intentional placement by RED.
