So every time I start a new game + after the trail cinematic and geralt starts to dream, a screen pops up and says I'm dead. Then I have to load back to my last save!
Yeah. What you do is that you take off all your gear and store everything you can (yes even the stuff you are wearing) in a storage-chest. Then start new game plus and you get your stuff back as soon as you get to the first town.

Thats the only time it happens, but it happened to a quite a few people. My buddy thought he had to wait for the next patch when it happened to him. It worked perfectly for me.
