Is it worth continuing to level up the sword in NG+, and can it last until the end of the game, DLCs included?

Definitely seems like a good idea to keep using this sword, yes.

Yes it is. The sword will stay the best silver sword when it comes to stats. Plus you don't have the need to smith any new silver sword in NG+
